There has been many notable published reports with supporting data on the true cost of defects.  The National Institute of Standard Technology (NIST) in 2002 stated:

"On average the time required to fix one defect in production is 15 hours as compared to a maximum of 5 hours in the coding stage"

The Defect Pareto chart summaries the root cause analysis conducted.  Presentation of defects found is classified by type and by the highest frequencies in which they occurred.

The data in the chart re-iterates the findings that the highest levels of defect are being introduced in the earliest phases of the delivery life cycle.
